The Mental Health Social Work Team
The Mental Health Social Work Team provides Assessment & Care management functions as defined by the Care Act 2014 to individuals over the age of 18 (although they also support young people in transition) who need support due to difficulties with their mental health.
The key areas of responsibility include:
- Provision of advice and information to individuals, families, and other professionals;
- Care Act Assessments of need and subsequent care planning;
- Joint working with colleagues from other sectors including health, other departments of the local authority, neighbouring local authorities, and the voluntary sector in order to support individuals;
- Commissioning of specific support/services to meet individuals identified. eligible needs;
- Reviews related to individuals assessed as having eligible needs;
- Adult Safeguarding referrals and subsequent Enquiries.
